Ce temps d\u2019\u00e9change, plac\u00e9 sous le signe de la concertation et de l\u2019\u00e9coute, a permis de rassembler de nombreux professionnels afin d\u2019identifier les enjeux du terrain et de faire \u00e9merger des pistes concr\u00e8tes d\u2019action.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Une volont\u00e9 affirm\u00e9e de mieux coordonner les soins<\/h2>\n\n\n\n<p>La r\u00e9union a \u00e9t\u00e9 introduite par <strong>Laurence Cass\u00e9<\/strong>, Membre du bureau de la CPTS Ouest H\u00e9rault, qui a rappel\u00e9 l\u2019objectif principal de ce temps fort :<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>\u00ab Afin de coordonner les soins sur notre territoire, nous avons r\u00e9uni les IDE exer\u00e7ant sur ce m\u00eame territoire afin d&rsquo;\u00eatre force de proposition dans leur exercice et d&rsquo;\u00eatre \u00e0 l&rsquo;\u00e9coute de leurs difficult\u00e9s. Nous remercions la mobilisation de ces professionnels de sant\u00e9 et leurs id\u00e9es que nous allons essayer de concr\u00e9tiser. \u00bb<\/em><\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Des \u00e9changes riches et des pistes \u00e0 concr\u00e9tiser<\/h2>\n\n\n\n<p>Les \u00e9changes ont port\u00e9 sur des th\u00e9matiques cl\u00e9s de l\u2019exercice infirmier en ville : continuit\u00e9 des soins, liens avec les structures hospitali\u00e8res, besoins en formation, coordination autour des patients complexes ou encore les attentes vis-\u00e0-vis de la CPTS en mati\u00e8re d\u2019appui au quotidien.<\/p>\n\n\n\n<p>Les professionnels pr\u00e9sents ont partag\u00e9 leurs <strong>pr\u00e9occupations concr\u00e8tes<\/strong>, mais aussi des <strong>propositions d\u2019actions collectives<\/strong> visant \u00e0 am\u00e9liorer la qualit\u00e9 et la fluidit\u00e9 du parcours de soins sur le territoire.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Un r\u00e9seau de partenaires mobilis\u00e9s<\/h2>\n\n\n\n<p>La CPTS Ouest H\u00e9rault a pu compter sur la pr\u00e9sence et le soutien de <strong>Dr Nicolas Breton<\/strong>, Pr\u00e9sident d\u2019<strong>Accueil Sant\u00e9 B\u00e9ziers<\/strong>, et de <strong>David Grandgirard<\/strong>, repr\u00e9sentant de <strong>Parsys T\u00e9l\u00e9m\u00e9decine<\/strong>, qui ont enrichi les \u00e9changes avec leurs expertises.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Et apr\u00e8s ?<\/h2>\n\n\n\n<p>Cette r\u00e9union marque une premi\u00e8re \u00e9tape vers la structuration d\u2019un <strong>groupe de travail infirmier territorial<\/strong>.
